気道吸引液中Matrix Metalloproteinase-9値と慢性肺疾患Ⅲ・Ⅲ ' 型の発症

　人工換気療法を受けた出生体重1,750g未満の児から生後24時間以内に採取された気道吸引液を検体として，matrix metalloproteinase（MMP）-9を測定した。慢性肺疾患ⅢあるいはⅢ ' 型（CLDⅢ・Ⅲ ' 型）を発症した児のMMP-9値（中央値［範囲］，115.3［45.0-635.0］ng/mg albumin；n=6）は，呼吸窮迫症候群と診断された児（22.7［0-195.3］ng/mg alb；n=9）あるいは新生児一過性多呼吸・未熟肺と診断された児（4.0［0-66.7］ng/mg alb；n=11）のMMP-9値に比して有意に高値であった。また，CLDⅢ・Ⅲ'型を発症した児のMMP-9値は，それ以外の型のCLDを発症した児のMMP-9値（12.4［0-66.7］ng/mg alb；n=6）に比しても有意に高値であった。CLDⅢ・Ⅲ ' 型では子宮内においてすでに肺の炎症が始まっているものと考えられており，肺障害を惹起する物質として顆粒球エラスターゼについての検討がなされてきたが，MMP-9もその発症に関与していることが考えられた。CLDの発症にあたっては肺に多種の蛋白分解酵素が出現することが考えられ，今後更に検討する必要がある。

　In this study, we measured the matrix metalloproteinase（MMP）-9 and interleukin（IL）-8 concentrations in the tracheobronchial aspirate of premature infants within 24 hours of birth. Twenty-six infants, who were admitted to our NICU from 1993 to 2000 and placed on a respirator, were included in this study. Six infants later developed chronic lung disease（CLD）following intra-uterine infection. Nine infants were diagnosed with respiratory distress syndrome（RDS）and 2 of them later developed CLD. Eleven infants were diagnosed with transient tachypnea of newborn（TTN）or premature lung and four of them later developed CLD. The MMP-9 concentrations in infants with CLD following intra-uterine infection（median［range］, 115.3［45.0-635.0］ng/mg alb；n=6）was significantly higher compared with the other 2 groups（22.7［0-195.3］ng/mg alb；n=9 and 4.0［0-66.7］ng/mg alb；n=11）. The MMP-9 concentrations in infants with CLD following intra-uterine infection　was also significantly higher compared with those in infants with CLD following RDS or TTN（12.4［0-66.7］ng/mg alb；n=6）. There was a significant relationship between the MMP-9 and IL-8 concentrations. MMP-9 seemed to play a role on the development of CLD following intra-uterine infection.
