生後まもなく,メチシリン耐性黄色ブドウ球菌(MRSA)敗血症を発症し,バンコマイシン(VCM)投与により敗血症は治癒したものの,約2ヶ月間の順調な経過のあと,MRSAによる多発性脳膿瘍を発症した超低出生体重児の1例を経験した。パルスフィールドゲル電気泳動およびHVR-PCR法(hypervariable region-PCR法)による細菌学的検索では,敗血症の起因菌であるMRSAと,脳膿瘍の起因菌であるMRSAが全く同じ菌株であることが同定された。すなわち,最初敗血症を起こしたMRSAが血行性に脳内に入り,多発性にMRSA感染巣が形成され,徐々に脳膿瘍へと進展,約2ヶ月間の一見順調な経過の後,発症した可能性も考えられる。従って,MRSA脳膿瘍のような重篤な合併症を防ぐためには,最初のMRSA敗血症の時,VCMをより長期に使用することも検討されなければならないと考えられた。また,このような疾患を防ぐためにはNICUにおいて,MRSA感染対策をしっかりと行っていくことも重要と思われた。 |

We report a extremely low birth weight infant who developed multiple Methicillin-resistant Staphylococcus aureus
(MRSA)brain abscesses. A 672g male infant was delivered at 29 weeks gestation. At 3days of age, blood investigation
revealed that C-reactive protein was positive and blood culture grew MRSA. He was treated with vancomycin for 10
days. But he developed multiple brain abscesses 2 month after an episode of MRSA sepsis. He was treated by
administration of teicoplanin for 29 days and surgical drainage. The MRSA grown from a blood culture in the initial
sepsis exhibited identical pulsed-field gel electrophoresis pattern and HVR-PCR pattern with that was isolated from brain
abscesses. When MRSA is the pathogen of the neonatal sepsis, vancomycin may be administerd for a longer period to
prevent severe complications such as a brain abscess. In addition, it is also thought that control and prevention of MRSA
in a NICU are very important to prevent such a serious MRSA infection. |
