チアノーゼ性心疾患の姑息術後にミルクアレルギー様症状を来たし遅発性の腸管狭窄を生じた2症例の検討

　チアノーゼ性心疾患に対するBlalock-Taussig（以下BT）短絡術の施行後に，遅発性の腸管狭窄をきたし腸管切除術を要した症例を2例経験した。術後に再開した経腸栄養の増量に伴い，症例1では炎症反応の遷延が，症例2では嘔吐と腹部膨満が遅発性に出現した。2例とも便中好酸球が陽性となりミルクアレルギーの関与を疑ったが，その後の手術所見で腸管狭窄が明らかとなった。病理組織所見では，腸管狭窄の原因として炎症後の二次的な変化が示唆された。チアノーゼ性心疾患に対するBT短絡術は，周術期に腸管虚血をきたす多くの要因がある。また，新生児における腸管の未熟性や虚血に伴う腸炎が抗原蛋白の侵入を容易にし，アレルギーによる二次性の炎症を惹起した可能性がある。BT短絡術後は，ミルクアレルギー様症状や遅発性の腸管狭窄が生じる可能性があり，細心の注意が必要である。

　We report 2 cases of infants with tardive intestinal stenosis that developed after placement of a Blalock-Taussig（BT） shunt for cyanotic congenital cardiac disease. Enteral feeding was restarted and increased more than 1 month after the BT shunt was inserted； subsequently, patient 1 exhibited continuously elevated C-reactive protein levels, and patient 2 developed vomiting and abdominal distention. In both cases, we suspected that these symptoms were due to a milk allergy because the stool smears repeatedly showed many eosinophils； however, irrigoscopy and surgical findings revealed intestinal stenosis. Pathological findings suggested secondary intestinal stenosis caused by inflammation. Patients in whom a BT shunt is being placed for cyanotic congenital heart disease are at an increased risk for bowel ischemia during the perioperative period. In addition, ischemic enterocolitis and the prematurity of the intestinal mucosa in neonates make allergen invasion easier； this allergen invasion may cause secondary inflammation associated with allergy. Therefore, careful attention should be paid to milk allergy-like symptoms and tardive intestinal stenosis that may develop after placing a BT shunt in infants with cyanotic congenital heart disease.
