青色発光ダイオード(LED)と緑色蛍光管(Green FL)による光療法の効果を後方視的に検討した。対象は出生体重1,062 〜2,800g の早産児43 例で,治療効果は照射時間あたりの血清総ビリルビン(TSB)値の低下率で比較した。TSB 低下率は青色LED のlow mode,high mode,Green FL の各群に有意差はなかった。しかし,lowmode では光療法開始12 時間のTSB 低下率は24 時間に比べて低い傾向がみられ(p =0.09),黄疸を軽減させる速効性は低いと考えられた。一方,光療法器としての治療効率(=TSB 低下率/ 放射照度)は光療法開始12 時間でGreen FL が青色LED のhigh mode よりも高い傾向を示した(p =0.08)ことから,黄疸の初期治療に対する治療効率の良い光療法はGreen FL であると考えられた。 |

We retrospectively analyzed the effects of phototherapy using blue light-emitting diode(LED)and green fluorescent
tubes(green FL).Subjects were 43 premature infants with a birth weight between 1,062 and 2,800g, and therapeutic
effects were compared in terms of the rate of reduction in total serum bilirubin(TSB)per exposure time. No significant
differences in TSB reduction rate were observed between the low-mode blue LED, high-mode blue LED, and green FL
groups. However, the low-mode tended to have a lower TSB reduction rate at 12 hours after initiation of phototherapy
compared to at 24 hours, and was not thought to rapidly alleviate jaundice. In addition, as the therapeutic efficiency of
the phototherapy device(=TSB reduction rate/irradiance)tended to be higher for green FL compared to high-mode
blue LED at 12 hours after initiation of phototherapy, green FL was thought to enable efficient phototherapy in the initial
treatment of jaundice. |
