角膜内皮炎を合併した先天性サイトメガロウイルス感染症の極低出生体重児例

　症例は在胎31週0日，出生体重1,100gの女児。出生時より汎血球減少と脳内石灰化を認め，血液検査より先天性サイトメガロウイルス（CMV）感染症と診断した。網脈絡膜炎はないが，角膜内皮炎を合併していた。6週間のガンシクロビル（GCV）とGCV点眼薬で角膜内皮炎は改善した。しかし投与終了後にCMVが再活性化して網脈絡膜炎を発症し，GCVおよびバルガンシクロビル（VGCV）で加療を行った。
　角膜内皮炎を伴う先天性CMV 感染症は報告がなく，貴重な症例と考えられた。GCVの投与で治癒したが投与終了後に網脈絡膜炎を発症した。GCVの投与期間は6週間では十分といえず，血漿中のCMV-DNA量を目安に，より長期の投与が望ましいと考えられた。

　We report a case of symptomatic congenital cytomegalovirus（CMV）infection with CMV corneal endothelitis in a very-low-birth-weight infant. Ganciclovir（GCV）was administered by intravenous drip infusion and eye drops for 6 weeks, and corneal endothelitis healed. However, CMV reactivated after termination of GCV therapy, and chorioretinitis developed. Treatment was restarted with GCV and valganciclovir（VGCV），improving chorioretinitis. Administration of GCV for 6 weeks appears insufficient to treat congenital CMV infection, and treatment by GCV or VGCV for longer terms while monitoring CMV-DNA levels in plasma.
