母体の摂食障害により頻回の無呼吸発作をきたし人工呼吸管理を要した新生児例

An Infant who Required Mechanical Ventilation Because of Frequent Apnea Attack Caused by the Maternal Eating Disorder

　We report a female infant who exhibited metabolic alkalosis with apnea due to respiratory compensation. The baby was born at gestational age of 37 weeks. Her birth weight was 2,420g, and Apgar score was 9 at 1 minute and 10 at 5 minutes. She suffered from apnea 12 hours after birth. Apnea was so frequent that mechanical ventilation was required. Blood examination revealed metabolic alkalosis with hypochloremia. Her mother had suffered from eating disorder and self vomiting. Maternal arterial blood gas analysis after 3 hours of delivery showed metabolic alkalosis with hypokalemia and hypochloremia. Metabolic alkalosis of the infant was caused by maternal eating disorder, and was compensated by hypoventilation and apnea. After treatment with mechanical ventilation and fluid therapy, metabolic alkalosis was gradually improved. If the maternal eating disorder is suspected, it is necessary to consider the possibility of metabolic alkalosis in infant as well as mother.

　摂食障害により嘔吐を繰り返し，代謝性アルカローシスを認めていた母体から出生し，無呼吸発作を繰り返したため，人工呼吸管理を要した新生児例を経験した。症例は生後18 時間の女児。在胎37 週5 日，出生体重2,420g，Apgar スコア生後1 分9 点，5 分10 点であった。出生直後は異常を認めなかったが，生後12 時間より無呼吸発作を認め，頻回であったため人工呼吸管理を行った。検査にて低Cl 血症を伴う重度の代謝性アルカローシスを認めた。母は摂食障害で自己誘発嘔吐を繰り返し，分娩後に代謝性アルカローシス，低K 血症，低Cl 血症を認めた。児の代謝性アルカローシスは母体の影響であり，呼吸性に代償するために無呼吸発作を引き起こしたと考えられた。人工呼吸管理と輸液で経過をみたところ，代謝性アルカローシスは改善した。母体に摂食障害が疑われる場合には，母児ともに代謝性アルカローシスを呈している可能性を考慮し，周産期管理を行う必要がある。
