片側腎低形成を有する新生児2 例を経験した。1 例は対側腎の代償性肥大を示し総腎機能は正常であったが,もう1 例は対側腎の低形成を伴い,胎児期から腎機能の低下を認め左右差のある両側腎低形成と考えられた。片側腎低形成では,対側腎が低形成なのか,代償性肥大を示すのかを確認することが生後の腎機能を予測する上で肝要である。胎児あるいは新生児の腎長径,腎容量の基準値に関するデータを用いて低形成の程度評価を簡便に行うことができる。超音波検査でスクリーニングを行い,症例を選んでCT もしくはMRI による正確な腎容量計測や腎シンチグラムによる機能評価が有用と考えられる。 |

We experienced two neonates with unilateral renal hypoplasia. In the first patient, contralateral kidney exhibited
compensatory hypertrophy with normal renal function. In contrast, the second patient had bilateral renal hypoplasia with
impaired renal function from an early fetal period. Prenatal detection of contralateral compensatory hypertrophy is useful
for prediction of postnatal renal function in patients with unilateral renal hypoplasia. Perinatal assessment of renal growth
is performed with reference values for kidney lengths and volumes of the fetus and newborn infants. After sonographic
assessment of renal dysplasia for screening, computed tomography and magnetic resonance imaging studies provide
precise renal volume measurements to predict the renal function in high-risk patients with unilateral renal hypoplasia. |
