18 トリソミーのため入院中,限局性脂肪肝を認めた乳児例を報告する。
症例は在胎39 週1 日,出生体重2,080g の女児。出生時,小脳低形成,口唇口蓋裂,重積指,心室中隔欠損症,左横隔膜挙上症を認め,染色体分析で18 トリソミーと診断。日齢60 の超音波検査で,偶然,肝左葉内側区に直径約1 cm の腫瘤状の高エコー病変を認めた。この高エコー病変は日齢86 のMRI ではT1 強調像で高信号,T1 強調像in-phase では肝実質と同じ信号強度,out-of-phase で低信号を呈し,脂肪を含むことが示唆された。この病変は圧排効果を示さず,血液生化学検査,血清CRP,血中腫瘍マーカーにも有意な異常はなく,生後11 カ月の超音波検査では消失していた。
授乳量の制限,中鎖脂肪酸油の添加といった18 トリソミーに合併した心疾患に対する栄養管理法などが本症の発症に関与した可能性があるが,本症と18 トリソミーの関連を解明するためにはさらなる検討が必要である。 |

We report a female infant with focal fatty liver. She was born at 39 weeks and 1 day of gestation, with a birth weight
of 2,080g. Multiple anomalies were present at birth, including cleft lip and palate, overlapping fingers, cerebellar
hypoplasia, and ventricular septal defect. Chromosomal analysis revealed the karyotype 47, XX, +18, and she was
diagnosed as having trisomy 18 syndrome. At 60 days of age, ultrasonography revealed an echogenic nodular lesion in
segment IV of the liver. Magnetic resonance imaging suggested that the lesion contained fat. The lesion exerted no mass
effect. Blood biochemistry tests, tumor markers, and serum C-reactive protein levels were unremarkable. The lesion
disappeared spontaneously by 11 months of age. These findings were consistent with focal fatty liver. Because of
congenital cardiac disease, her feeding volume was restricted and medium-chain triglycerides were added to her milk
when the hepatic lesion was found. The nutritional management possibly caused focal fatty liver in this infant. Further
studies are necessary to elucidate the etiological relationship between focal fatty liver and trisomy 18 syndrome. |
