2008年に承認された遺伝子組換え型ヒトトロンボモジュリン製剤(rTM)は,成人の播種性血管内凝固症候群(DIC)での有効性が多数報告されているが,極低出生体重児のDICに対する効果についての報告はまだない。今回,当院での極低出生体重児DIC に対するrTMの治療成績を,当院での従来の治療方法と比較して検討したので報告する。対象はDICと診断した極低出生体重児の23例で,ナファモスタットメシル酸塩で治療したコントロール群(C群)8例と,rTMで治療を行ったトロンボモジュリン群(T群)15例である。C群と比較して,T群で濃厚血小板(PC),新鮮凍結血漿(FFP)の投与日数が有意に少なく,活性化部分トロンボプラスチン時間(APTT),フィブリン分解産物(FDP)の改善率も有意に大きかった。rTMは極低出生体重児DICの治療において,従来の治療法よりもより有用であることが示唆された。 |

Although numerous recent reports suggested that recombinant human thrombomodulin(rTM),which was approved
in Japan in 2008, is effective against disseminated intravascular coagulation(DIC)in adults, little is known about its
efficacy in very low birth weight infants(VLBWIs).To evaluate the efficacy of rTM against DIC in VLBWIs, we
retrospectively analyzed the infants with DIC by comparing those treated with rTM(T group, n=15)and those treated
with nafamostat mesilate(C group, n=8).The analysis revealed that T group required significantly fewer days for
platelet concentrate transfusion and for fresh frozen plasma transfusion(p < 0.05)than C group, and also demonstrated
greater improvement in values of activated partial thromboplastin time and fibrin degradation products(p < 0.05).These
results suggest that rTM may be more effective against DIC in VLBWIs in comparison with nafamostat mesilate. |
