乳児は生後5 か月までの間,自発運動と呼ばれる運動を通し運動学習をしている。極低出生体重児はその未熟性から抗重力運動が苦手であり,正期産児と異なるパターンを示すとされる。今回我々は極低出生体重児の自発運動の抗重力運動の特徴を定量化すること,また,極低出生体重児の抗重力運動の特徴を明らかにすることを目的に3次元動作解析システムを用い,自発運動の解析を行った。対象は極低出生体重児(VLBW 群)7 名,および正期産児(Control 群)10 名であった。自発運動の上下肢左右接近距離(最小値),運動の高さ(最大値)はVLBW 群とControl 群とで有意差を認めなかった。しかし,上下肢左右接近距離(平均値)はVLBW 群で有意に大きく,運動の高さ(平均値)は有意に小さく,抗重力運動が少ないことが明らかとなった。VLBW 群では早産により早期に重力の影響を受けることや医療的処置の影響で抗重力運動の機会が少ないことが結果に影響していると考えられた。 |

Objective:To investigate the characteristics of anti-gravity movements, also known as spontaneous movements, in very
low birth weight(VLBW)infants at one month corrected age.
Methods:Subjects were seven VLBW infants who were born at Sapporo Medical University Hospital and ten full-term
infants(Control). Spontaneous movements were analyzed using a three-dimensional motion analysis system.
Result:There was no significant difference in the minimum of approached distance of limbs and the maximum of the
height of movements. However, in the group of VLBW, the average of approached distance of limbs was significantly
larger(p<0.05), the average of the height of movements was significantly lower(p<0.05)than in the control group.
Conclusion:We reached a conclusion that spontaneous movement is decreased in VLBW infants because the effect
of the gravity in the extra-uterine environment is far greater to them, and there is less chance of moderate movement
under the influence of medical treatment. |
