NICU に入院後,点滴が終了し経腸栄養が確立した児で,低血糖を回避するため授乳間隔を2 時間毎(12 回乳)に設定していた早産低出生体重児10 例を対象とした。血糖が安定したと判断して授乳間隔を3 時間毎(8 回乳)に変更する際に,持続血糖測定(continuous glucose monitoring:CGM)を行い,血糖値の「授乳区間変動幅」を記録から算出した。さらに,8 回乳に変更後1 ~ 4 週間で再度CGM を行い,「授乳区間変動幅」の推移を比較した。結果は,12 回乳,8 回乳変更直後,8 回乳変更後1 ~ 4 週間でそれぞれ,26.4±12.7(Mean±SD)mg/dL,55.4±19.1mg/dL,38.7±14.7mg/dL となり,8 回乳に変更することで,血糖値の変動幅は有意に増大したが,変更後1 ~ 4 週間経過すると有意に減少し,Scheffe 法による多重比較では12 回乳と8 回乳で有意な差を認めなくなった。従来の定時的採血による血糖管理と比較して,CGM は血糖値の変動が検討できる点で,新生児領域においても有用であることが示唆された。

We studied 10 preterm infants after full enteral feeding at NICU who were scheduled to change the feeding interval
from 2 to 3 hours for the management of blood glucose levels. We conducted CGM and calculated glycemic fluctuations
before and after the change of feeding intervals. Furthermore, we reexamined CGM 1 ~ 4weeks after the change of
the feeding interval. As a result, glycemic variability revealed just before, just after, and 1 ~ 4 weeks after the change
of feeding intervals, 26.4±12.7(mean±SD)mg/dL, 55.4±19.1mg/dL, and 38.7±14.7mg/dL, respectively. Glycemic
variability was significantly increased soon after the change of feeding interval and significantly decreased after more 1
~ 4 weeks, so it did not differ significantly between 2-hour feeding interval and 3-hour feeding interval of 1 ~ 4 weeks
after the change.
In comparison with blood glucose monitoring by routine point-of-care testing, CGM could record glycemic fluctuations
as well as hypoglycemic events. CGM revealed the new usefulness in the neonatal field. |
