目的:照射赤血球濃厚液-LR「日赤」(Irradiated Red Cells Concentrate, Leukocytes Reduced, NISSEKI:Ir-RCC),新鮮凍結血漿-LR「日赤」(Fresh Frozen Plasma, Leukocytes Reduced, NISSEKI:FFP),照射濃厚血小板-LR「日赤」(Irradiated Platelet Concentrate, Leukocytes Reduced, NISSEKI:Ir-PC)を種々の比率で混合した血液(混合血)の性状を確認し,交換輸血使用血としての適格性を評価する。
方法:混合比の影響は採血後4 日のIr-RCC を用い,Ir-RCC:FFP=1:1,2:1,Ir-RCC:FFP:Ir-PC=4:2:1 の比で混合し検討した。Ir-RCC の採血後日数の影響は採血後4 日,13 日,20 日のIr-RCC を用い,Ir-RCC:FFP=2:1 の比で混合し検討した。
結果:Ir-RCC:FFP=1:1,2:1,Ir-RCC:FFP:Ir-PC=4:2:1 それぞれの混合比における平均値はヘマトクリット(Ht)が27.5%,37.6%,32.7%,総蛋白(TP)が4.7g/dL,3.8g/dL,4.3g/dL,K が8.9mEq/L,12.5mEq/L,10.5mEq/Lであった。採血後4 日,13 日,20 日でのK の平均値はそれぞれ12.5mEq/L,24.0mEq/L,27.6mEq/L であった。
考察:混合血はHt,TP が低値,K が高値など照射合成血液-LR「日赤」に比し,交換輸血使用血として注意すべき点が多い。従って,照射合成血液-LR「日赤」の入手が間に合わない場合に限り使用するのが良く,使用時は副作用に配慮が必要である。 |

Objective:To suitability the eligibility for exchange transfusion of blood component ratios including “Irradiated Red Cells Concentrate, Leukocytes Reduced, NISSEKI” (Ir-RCC), “Fresh Frozen Plasma, Leukocytes Reduced, NISSEKI” (FFP), and “Irradiated Platelet Concentrate, Leukocytes Reduced, NISSEKI”( Ir-PL) at various ratios. Methods:Effects of different component blood ratios were evaluated including Ir-RCC:FFP=1:1, Ir-RCC:FFP=2:1, and Ir-RCC:FFP:Ir-PL=4:2:1 using Ir-RCC of day 4. The effect of age of Ir-RCC was evaluated by using RCC aged 4, 13, and 20 days after production at a ratio of Ir-RCC:FFP=2:1. Results:The means of hematocrit( Ht) were 27.5%, 37.6%, 32.7%;total protein concentrations 4.7 g/dL, 3.8 g/dL, 4.3 g/ dL;and potassium( K) concentrations 8.9 mEq/L, 12.5 mEq/L, 10.5 mEq/L, respectively, at the ratios of Ir-RCC:FFP= 1:1, Ir-RCC:FFP=2:1, Ir-RCC:FFP:Ir-PL=4:2:1. The means of K concentration of the mixed blood preparations with RCC aged 4, 13, and 20 days were 12.5 mEq/L, 24.0 mEq/L, and 27.6 mEq/L, respectively. Discussion:The mixed blood preparations examined had disadvantages;low protein concentration;low Ht, and high K concentration for exchange transfusion, compared with “Irradiated Blood for Exchange Transfusion, Leukocytes Reduced, NISSEKI” (Ir-BET). Use of the mixed blood preparations examined in this study should be made only if Ir-BET is unavailable, with careful consideration of side effects. |
