背景:早産児における甲状腺機能と発育および発達の関連性は明らかでない。
目的:甲状腺機能が生後早期の発育および発達に与える影響を検証する。
方法:在胎33 週未満の児の生後2 週,4 週の血清TSH 値およびFT4 値とNICU 退院時の体重,頭囲(パーセンタ
イル値)およびDubowitz スコアとの関連を後方視的に検討した。
結果:頭囲に関する重回帰分析では生後2 週の血清FT4 値で有意差を認めた(β=0.343,P=0.04)。体重および
Dubowitz スコアに関する重回帰分析では甲状腺機能と有意差を認めなかった。退院時の頭囲は性別(女児:β=
-0.425,P=0.002)とfull-feeding に達した日数(生後10 日以降:β=-0.295,P=0.04)が関連していた。
結語:早産児の生後2 週の血清FT4 値は短期間の頭囲の発育と関連する可能性があった。 |

Background:It is not clear whether thyroid function in preterm infants is related to growth and neurocognitive development.
Objective:The aim of this study was to verify the effect of thyroid function on short-term growth and development.
Methods:We retrospectively analyzed the correlation between serum thyroid-stimulating hormone( TSH)/free thyroxine
(FT4) levels in preterm infants( <33weeks) 2/4 weeks after birth and growth( weight and head circumference)/neurocognitive
development( Dubowitz score) around full term of corrected age.
Results:A significant relationship between head circumference and serum FT4 levels 2 weeks after birth was observed
in multiple regression analyses (β=0.343, P=0.04). There was no significant relationship between weight or Dubowitz
score and thyroid function in multiple regression analyses. Increases in head circumference were related to sex( female:
β=-0.425, P=0.002) and time to achievement of full enteral feeding( over ten days after birth:β=-0.295, P=0.04).
Conclusions:Serum FT4 levels 2 weeks after birth may be related to short-term head circumference growth in preterm
infants. |
