今回我々は,high flow nasal cannula(以下HFNC)の離脱が困難な18 トリソミーの児に対して,HFNC で在宅
医療に移行できた貴重な症例を経験した。以前まで在宅で使用可能な新生児向けのHFNC はなかったが,2017 年2
月より新生児でも在宅で使用できるHFNC が利用可能となった。本症例は退院時の体重が2,294g と低体重であった
ため,マスクによる人工呼吸療法はマスクのフィットが難しくリーク量が多くなり有効な換気が得られないことが
予測された。そのためマスク以外のインターフェイスが望まれHFNC で退院することを目標にし,実際に有効な換
気を得られ在宅医療に移行できた。乳幼児の在宅医療にHFNC が導入される可能性が広がる点で,今後につながる
症例だと思われる。 |

Here, we report on a trisomy 18 infant receiving high-flow nasal cannula (HFNC) oxygen therapy at home. Although
HFNC therapy in neonates is usually performed at hospitals, HFNC equipment for use at home became available in February
2017. Because the body weight of the patient at the time of discharge was low( 2,294g), artificial respiration therapy
at home was expected to be impractical due to the lack of a suitable mask size and effective ventilation. Accordingly, the
team searched for an interface other than a mask and aimed to discharge the patient with HFNC. The equipment provided
effective ventilation and the patient was able to shift to home care. We expect that the practice of providing HFNC for
home care for infants will be expanded in the future. |
