血栓を形成し自然閉鎖した新生児孤立性動脈管瘤の3 例

Ductus Arteriosus Aneurysm associated with Thrombus Formation in the Neonates：Three Case Reports

　血栓を形成し自然閉鎖した新生児孤立性動脈管瘤（ductus arteriosus aneurysm：DAA）3 例の閉鎖過程を心エコー図で経時的に観察した。症例1 は在胎35 週2 日の胎児超音波検査で拡張した動脈管を認めていた。日齢0 にDAA は最大径を呈し，日齢4 までに急速に縮小したが，その後の縮小はみられなかった。日齢7 にはDAA 内壁側に層状の血栓形成を認め，日齢18 に閉鎖した。症例2 は日齢1 にDAA は最大径となり，日齢2 に肺動脈側閉鎖後軽度縮小したが，その後の縮小は認めなかった。症例3 は日齢4 に血栓性閉鎖を認めたが，DAA 径にあまり変化は認めなかった。DAA が血栓を形成し自然閉鎖する過程において異なったパターンがみられた。肺動脈側の閉鎖が遅れた場合，瘤は比較的早期に一旦縮小するが，その後の縮小はみられなかった。一方，肺動脈側が早期に閉鎖した場合，瘤内は血栓で充満され，瘤の縮小は軽度ないしは認めなかった。

　We observed the spontaneous resolution of ductus arteriosus aneurysm（ DAA） associated with thrombus formation in three neonates using two-dimensional Doppler echocardiography（ 2DD）. In the first case, fetal 2DD at 35 weeks of gestation showed ductal dilatation. On the first day of life, the DAA was observed at a maximum internal diameter and rapidly diminished by Day 4；however, no further resolution was observed thereafter. Repeat 2DD showed layered thrombus formation on the inner wall of the DAA on Day 7. The DAA was filled with thrombus and closed on Day 18. In the second case, 2DD on Day 1 showed the DAA at its maximum size. On the following day, although the DAA diminished mildly after closing on the pulmonic end, no further resolution was observed thereafter. In the third case, 2DD showed thrombotic occlusion on Day 4, while the DAA diameter did not show any significant changes. The process of the DAA developing a thrombus and spontaneously resolving followed different patterns. Delayed closure at the pulmonic end of the DAA led to relatively rapid constriction；however, no further resolution was observed thereafter. The early closure at the pulmonic end of the DAA led to it being filled with thrombus, revealing mild or no constriction of the DAA.
