目的:早産児における生後の血清IGF-I 値と短期間の発育との関連性を明らかにする。
方法:在胎32 週未満の児の生後3 週および5 週の血清IGF-I 値とNICU 退院時の体重との関連を後方視的に検討し
た。退院時体重と出生体重のSD スコアの差をΔSD スコアとし,ΔSD スコア≥ 0 を体重増加良好群,ΔSD スコア
<0 を体重増加不良群とした。
結果:多変量解析では体重増加不良と生後5 週の血清IGF-I 値は有意な関連を認めたが(OR 0.79,95%CI=0.64 ~
0.97,P=0.023),生後3 週の測定値は有意差を認めなかった(OR 1.04,95%CI=0.90 ~ 1.21,P=0.56)。
結語:早産児の血清IGF-I 値は生後5 週の測定値が短期間の体重増加の予測に有用である可能性を示した。 |

Objective:The aim of the present study was to verify the relationship between insulin-like growth factor I( IGF-I) levels
and short-term growth in preterm infants after birth.
Methods:We retrospectively analyzed serum IGF-I levels in preterm infants( <32 weeks) at 3 and 5 weeks after birth
and weight at NICU discharge. Birthweight( SD score) was subtracted from weight at NICU discharge, and ΔSD scores
were calculated. Infants with ΔSD score ≥ 0 were regarded as having good weight gain, and infants with ΔSD score <
0 as having poor weight gain.
Results:In multivariate analysis, poor weight gain was significantly correlated with serum IGF-I values at 5 weeks after
birth( OR 0.79, 95%CI=0.64-0.97, P=0.023), but not values at 3 weeks after birth( OR 1.04, 95%CI=0.90-1.21, P=0.56).
Conclusion:The results demonstrate the effectiveness of serum IGF-I measurements at 5 weeks after birth in predicting
short-term weight gain. |
