新生児期発症の先天性梨状窩瘻に対する 10％硝酸銀水溶液による化学的焼灼術の試み

　梨状窩瘻は第 3 ～ 4 咽頭嚢遺残による先天異常である。治療方法は瘻管摘出術と化学的焼灼による瘻管焼灼術に大別される。今回我々は新生児期発症の先天性梨状窩瘻治療として 10％硝酸銀水溶液による化学的焼灼術を実施し，良好な結果を得た。症例は在胎 37 週 4 日で出生した女児。日齢 5 に頸部腫脹および呼吸障害を認めたため同日当院へ搬送された。頸部 CT 検査で左頸部膿瘍による気道圧排を認め，経皮的膿瘍ドレナージ術を実施した。日齢 16 に喉頭直逹鏡を使用し病変部を観察したが瘻孔は確認できなかった。日齢 38 に再度瘻孔を確認した上で 10％硝酸銀水溶液を用いて化学的焼灼術を実施した。その後 CT 検査で頸部に新たな気泡を認めたため日齢 54 に同様の手術を行い，増悪なく日齢 62 に退院となった。新生児期から乳児期早期の梨状窩瘻に対する治療として化学的焼灼術は，侵襲が小さく合併症の少ない治療方法の一つである。

Piriform sinus fistula is a congenital anomaly due to a remnant of the third or fourth pharyngeal pouch. It is commonly treated by fistulectomy or chemical cauterization. Here we report a case of neonatal-onset congenital piriform sinus fistu-la successfully treated by chemical cauterization with a 10％ silver nitrate solution. The patient was a female infant with a gestational age of 37 weeks and 4 days. She was transported to our hospital at 5 days of age because neck swelling and respiratory problems were observed. A CT scan of the neck revealed airway constriction due to left neck abscess, and abscess drainage was performed. At 16 days of age, the lesion was observed using direct laryngoscope and no fistula was found. After confirming the presence of fistula at 38 days of age, chemical cautery was performed using a 10％ silver ni-trate aqueous solution. Thereafter, a CT examination revealed new air lesion in the neck, and the same operation was performed again at 54 days of age. She was discharged at 62 days of age without deterioration. The present case high-lights the usefulness of chemical cautery as a less invasive, low-complication-risk treatment option for piriform fistula from the neonatal period to the early infancy period.
