緒言:低出生体重児では肝芽腫発症の頻度が高いことが報告されている。今回,全国の主な新生児医療施設における早産児,低出生体重児の肝芽腫発見のための定期的な検査について実態調査を行った。
方法:日本周産期・新生児医学会の周産期専門医(新生児)研修施設を対象にアンケートを行った。
結果:466 施設中296 施設(63.5%)から回答を得た。早産児,低出生体重児の肝芽腫発見のための定期的検査を
行っていると回答した施設は9 施設(3.0%)であった。検査対象の在胎期間は32 週未満が1 施設,出生体重は
1,000g 未満が1 施設,1,500g 未満が6 施設であった。検査方法は血清アルファフェトプロテイン測定のみが4 施設,
超音波検査のみが2 施設,血清アルファフェトプロテイン測定と超音波検査が2 施設であった。
結論:肝芽腫発見のために定期的検査を行っている施設は少なく,検査方法も施設によって異なっていることが判
明した。 |

Low-birth-weight infants are at increased risk of developing hepatoblastoma compared with normal-birth-weight infants.
We conducted a nationwide survey of the screening programs for early detection of hepatoblastoma in preterm and/or
low-birth-weight infants. Questionnaires were sent to 466 training hospitals for neonatologists in Japan, and responses
were obtained from 296 hospitals( 63.5%). Only nine( 3.0%) hospitals had screening programs for the early detection of
hepatoblastoma in preterm and/or low-birth-weight infants. The screening program was applied to infants with a gestational
age of <32 weeks at one hospital, and to infants with a birth weight of <1,000 g at one hospital and <1,500 g at
six hospitals. The screening methods comprised measurement of serum α-fetoprotein at four hospitals, ultrasonography
at two hospitals, and both methods at two hospitals. This study revealed that few hospitals have screening programs for
the early detection of hepatoblastoma in preterm and/or low-birth-weight infants. The methods used in screening programs
varied considerably among the hospitals. |
