Hotz 床を装着した口唇顎裂,口蓋裂児の経口哺乳確立時期の検討
―片側と両側の比較―

 口唇口蓋裂の片側,両側の差がHotz 床装着後の哺乳に影響するかについて一定の見解はない。口唇口蓋裂等の診 断でHotz 床を装着した在胎36 週以降の児で,重篤な合併症のない児を対象とし,Hotz 床装着後の哺乳に影響する 因子を調査した。Hotz 床を装着した口唇口蓋裂児を片側群,両側群に分け,周産期背景および新生児期の経過を比 較した。口蓋裂・口唇口蓋裂40 例が対象となり,片側群27 例,両側群13 例であった。Hotz 床装着から哺乳確立 (哺乳量100mL/kg/ 日)までの日数(中央値[範囲])は,片側群(3[1 ~ 9])と両側群(3[0 ~ 13])で差はな かった。一方で,哺乳確立までの日数と在胎週数には有意な負の相関が認められ(p<0.01),片側,両側を問わず, Hotz 床装着後はより成熟した児が早期に哺乳確立できることが示唆された。
To investigate the associated factors of oral feeding in infants with cleft lip and palate who were using a Hotz-type plate, we retrospectively reviewed clinical data of such infants who were admitted to the neonatal intensive care unit. Infants with serious congenital anomalies and who were born before 36 gestational weeks were excluded. Patients were classified into unilateral and bilateral cleft lip and palate groups. We investigated the duration between wearing the Hotz plate and attaining full oral feeding (defined as a milk volume intake of 100mL/kg/day). During the 13-year study period, 40 patients( 27 unilateral, 13 bilateral) with cleft lip and cleft palate were included in the present study. No significant difference in the duration was found between unilateral( median[ range]:3[ 1-9]) and bilateral groups( 3[ 0-13]). A significant negative correlation was observed between gestational age and the duration( correlation coefficient -0.48, p<0.01). The duration between wearing a Hotz-type plate and attaining full oral feeding was associated with the gestational age, regardless of cleft type.
