Skimmed breast milk による母乳栄養を行った先天性乳糜胸の1 例

　近年海外では，乳糜胸における内科的治療として脂肪減量母乳（skimmed breast milk：SBM）の使用が報告されている。難治性の先天性乳糜胸に対してSBM による母乳栄養を取り入れ自宅退院に至った症例を経験した。症例は在胎31 週で出生した男児である。出生時より両側乳糜胸を認めた。胸水貯留は絶食により一時改善したが，少量の母乳栄養を開始したところ再燃した。Manual separation による脂肪減量方法で作製したSBM を使用したところ胸水は出現しなかった。1 日の栄養の半量をSBM に置き換え，十分な体重増加を得ることができた。母乳成分分析を行い，SBM の含有タンパクは一定に保たれ適切に脂肪減量ができていることを確認した。母乳栄養は人工栄養と比較して多くの利点を有し育児支援にも繋がる。自宅で作製可能なSBM は退院後も継続可能であり，乳糜胸患児の栄養法として今後有効な選択肢となり得る。

In recent years, skimmed breast milk（ SBM） has been used an alternative to medium-chain triglyceride milk formula as a medical treatment supplementation for chylothorax worldwide. We present a case of congenital chylothorax supplemented with homemade SBM, resulting in a successful discharge. The patient was a male infant born at 31 weeks of gestation. Chylothorax on both sides was observed at birth. Pleural effusion improved with fasting, but relapsed soon after breastfeeding in small amounts was started. As an alternative treatment, SBM prepared by “manual separation,” a fat removal method, was introduced, which resulted in the remission of pleural effusion. The SBM was analyzed using a breast milk component analyzer, which confirmed that the target level of defatting was achieved. The protein content remained stable；thus, we concluded that the SBM had an effective nutritional benefit. We replaced half of the daily nutrient intake with SBM, and the patient had no relapse of pleural effusion. It is widely known that breastfeeding has many advantages over formula milk supplementation and helps to improve childcare. By introducing self-made SBM, we can expand the choice of nutritional supplementation for children with chylothorax.
