横隔膜の電気的活動(electrical activity of diaphragm:Edi)をトリガーとする神経調節性補助換気(neurally
adjusted ventilatory assist:NAVA)は人工呼吸器との同調性を改善し,呼吸パターンが不規則な新生児の呼吸管
理に適する。先天性横隔膜ヘルニア(congenital diaphragmatic hernia:CDH)術後ではEdi の検出への影響が懸念
されてきた。しかし,近年ではNAVA での管理が可能とする報告が散見される。我々はCDH 術後の3 例にNAVA
を使用した。Edi の検出が困難であった1 例は中枢神経系の合併症を有していた。パッチ閉鎖術を要した左全欠損
の症例を含む2 例はNAVA での管理が可能で,吸気圧や酸素濃度を漸減することができた。NAVA はgentle ventilation
が求められるCDH 術後に適した人工呼吸管理と考えられた。 |

Neurally adjusted ventilatory assist( NAVA) is a method of artificial ventilation that utilizes electrical activity of the diaphragm(
Edi) as a trigger for respiratory support. NAVA can improve patient-ventilatory synchrony, which is especially
appropriate in neonates because of their irregular breathing patterns. There has been concern about the detection of Edi
signals after congenital diaphragmatic hernia( CDH) repair. However, successful management with NAVA after CDH repair
has recently been reported. In our institution, 3 neonates after CDH repair have received respiratory support with
NAVA. In one case there was difficulty in detecting Edi signals, and central nervous system complications. In the other 2
cases, including one with total agenesis of left-sided diaphragm that required patch closure, we successfully used NAVA,
with a lower peak inspiratory pressure and reduced oxygen requirement. NAVA is an appropriate method of ventilation
management in neonates after CDH repair who require gentle ventilation. |
