【背景】極低出生体重児の胎便関連性腸閉塞症の診断・治療目的に行われる水溶性消化管造影剤(ガストログラフィ
ン ®)胃内注入・注腸療法にはヨード過剰による甲状腺機能低下症の危険性があるが,十分に検討されていない。
【方法】水溶性消化管造影剤療法を行った極低出生体重児(ガストロ群)と在胎週数および出生体重を合わせた対照
群の甲状腺刺激ホルモン(TSH)値等を後方視的に比較した。
【結果】ガストロ群(5 例)のTSH 値は,対照群(15 例)と比較し,投与数日後には高値で(中央値6.4μIU/mL
vs 3.2μIU/mL,p=0.01),投与1 か月後も高値であった(中央値14.1μIU/mL vs 3.0μIU/mL,p=0.01)。
【結論】水溶性消化管造影剤療法は,甲状腺機能低下症を来す可能性がある。水溶性消化管造影剤療法を受けた新生
児は全例,投与1 か月後頃にTSH の評価を行うべきである。 |

Background:Water-soluble contrast medium (Gastrografin ®) therapy, when administered intragastrically or as enema,
may be used for the diagnosis and treatment of meconium-related ileus in very-low-birthweight infants. Although there is
a theoretical risk of hypothyroidism due to its high iodine content, this has not been sufficiently studied.
Methods:A retrospective study was conducted to compare thyroid-stimulating hormone( TSH) levels and other parameters
between very-low-birthweight infants who had undergone water-soluble contrast medium therapy versus gestational
age- and birth weight-matched controls.
Results:The TSH levels of the Gastrografin ® group( n=5) were significantly higher than those of the control group( n
=15) after a few days post-administration( median 6.4μIU/mL vs. 3.2μIU/mL, p=0.01) and at one-month post-administration(
median 14.1μIU/mL vs. 3.0μIU/mL, p=0.01).
Conclusion:Water-soluble contrast medium therapy for meconium-related ileus may cause hypothyroidism. Hence, all
neonates who have received water-soluble contrast medium therapy should undergo TSH evaluation at one-month postadministration. |
