COVID-19 流行で当院でも面会制限を余儀なくされたため,当院NICU では試験的に画像・情報配信を併用した 家族サポートを行い拡充してきた。配信の効果を検討するため,家族へのアンケート,退院時・退院後の母乳率, 産後1 週間の母のエジンバラ産後うつ病質問票(Edinburgh postnatal depression scale:EPDS)の結果を検討した。 アンケートでは,安心を感じ,スタッフとコミュニケーションが取りやすく,児のことを考える機会となるという 好意的な意見が多かった。在胎32 週未満または出生体重1,500g 未満の児の退院時母乳率は改善し,退院後の母乳 率も同様に上昇,EPDS 高値となる率は低下した。画像や情報に加えスタッフとのやりとりを通し,母が児に主体 的に関わっている感覚を持てる可能性,家族親族にも共有することで母子のサポート体制が強固になる可能性が示 唆され,画像・情報配信は家族に対する一定のサポート効果をもたらすことが示された。 |
We used image and essential tip streaming for family support in our neonatal intensive care unit during restricted family hospital visits due to the COVID-19 pandemic. In this study, we analyzed questionnaire data, breastfeeding rate at and after discharge, and mother’s Edinburgh Postnatal Depression Scale( EPDS) score at 1 week postpartum, to examine the effects of the streaming. The results of the questionnaire revealed that families felt reassured, had easier communication with staff, and had an opportunity to feel connected with the child through the streaming. The breastfeeding rate at discharge in infants born under 32 weeks of gestation or under 1,500g birth weight improved with streaming. The breastfeeding rate after discharge increased as well, and the proportion of mothers who had a high EPDS score decreased. Taken all together, the results indicated that the mothers had positive involvement with their babies through communication with hospital staff, in addition to seeing images and information. Furthermore, sharing the contents and communication with other family members and relatives can lead to them showing attitudes of solid support toward the mother and child. This streaming of images and essential tips can have a certain supportive effect on mothers and families, and can be a strong option in family support. |
