胎児超音波検査で大きく突出した特徴的な耳介を認めた9pトリソミーの1 例

A Case of 9p Trisomy with Characteristic Ears Detected by Fetal Ultrasound

胎児超音波検査等で特徴的な耳介変形を認めた9p トリソミーの1 例を経験した。症例は在胎36 週1 日で出生 した女児である。胎児期に，胎児発育不全（fetal growth restriction：FGR）のほか，頭蓋から外側に大きく突出 した特徴的な耳介が指摘されていた。出生後，大きく外側に突出した折れ耳，耳介低位，小頭症といった特徴的な 外見に加え，哺乳緩慢や体重増加不良，心房中隔欠損症，両側難聴，痙攣群発，多発性の椎体低形成と多臓器にわ たる合併症を認めたことから染色体疾患が疑われた。染色体検査にて9p トリソミーと診断した。特徴的な外見や 多臓器に及ぶ合併症に加え，胎児超音波検査等の所見も本疾患を疑う手がかりとなる可能性がある。

We report a female case of 9p trisomy with characteristic ears detected by fetal ultrasound. Fetal ultrasound revealed growth restriction and characteristic ears that protruded from the head. The patient was born at 36 weeks and 1 day of gestation with microcephaly and large, low-set and overfolded ears. In addition, she showed poor feeding, poor weight gain, atrial septal defect, bilateral auditory disturbance, clusters of convulsions, and multiple hypoplasias of vertebral bodies. A chromosomal abnormality was suspected, and we diagnosed 9p trisomy. In addition to characteristic appearances and multi-organ complications, fetal ultrasound may be useful in the diagnosis of this abnormality.
