SADDAN(severe achondroplasia with developmental delay and acanthosis nigricans)はタナトフォリック骨異
形成症(thanatophoric dysplasia:TD)の責任遺伝子でもあるFGFR3(fibroblast growth factor receptor 3)遺伝
子の変異による非常に稀な疾患で,臨床像はTD と酷似しているが,呼吸障害は軽度で,下腿の弯曲を認める点が
特徴的である。本症例は,胎児CT 検査で大腿骨短縮,ベル型胸郭などの所見よりTD と考えていたが,出生後の
呼吸障害は軽症で,FGFR3 遺伝子のLys650Met 変異を認めたことよりSADDAN と診断した。TD と共通する骨所
見に加え,胎児MRI 検査での肺成熟度,比較的軽度の呼吸障害,下腿の変形に着目できれば,より早期にSADDAN
を疑うことができたと考えられる。 |
SADDAN (Severe Achondroplasia with Developmental Delay and Acanthosis Nigricans) is an extremely rare disorder
caused by mutations in the FGFR3 (Fibroblast Growth Factor Receptor 3) gene, which is also responsible for Thanatophoric
Dysplasia( TD). While clinical features of SADDAN closely resemble those of TD, SADDAN can be characterized
by milder respiratory impairment and posterior tibial bowing. In this case, fetal CT scans suggested TD due to findings
such as femoral shortening and a bell-shaped thorax. However, based on mild postnatal respiratory impairment and the
Lys650Met mutation in the FGFR3 gene, we confirmed a diagnosis of SADDAN. In addition to the bone findings common
to TD, lung maturity observed in fetal MRI, focusing on relatively mild respiratory impairment and deformities of the
lower legs could have led to an earlier suspicion of SADDAN. |
