SGA(small for gestational age)児において母体の妊娠高血圧症候群(hypertensive disorder of pregnancy:
HDP)の有無が3 歳時の身長に与える影響について検討した研究は少ない。SGA 児において,修正3 歳時身長のZ
スコアが-2.0 未満の児の割合および-2.5 未満の児の割合について,HDP を発症した母体より出生した児(HDP 群)
とHDP を発症していない母体から出生した児(non-HDP 群)を比較して検討した。2011 年から2021 年に当院に
入院した在胎23 週から在胎32 週の児のうち,修正3 歳時身長の記録がある103 例を対象とし,HDP 群とnon-HDP
群の修正3 歳時身長のZ スコアを比較した。修正3 歳時身長のZ スコアが-2.0 未満の児の割合は,HDP 群で23.3
%,non-HDP 群で30.2%であった。修正3 歳時身長のZ スコアが-2.5 未満の児の割合は,HDP 群で13.3%,non-
HDP 群で20.9%であった。HDP 群とnon-HDP 群間において,身長がcatch-up しなかった児の割合に有意な差はな
かった。本研究では,在胎32 週以下のSGA 児において,母体HDP の有無と修正3 歳時身長がcatch-up しなかっ
た児の割合との間に,有意な関連を示さなかった。 |
Few studies have examined the effect on the height of small for gestational age (SGA) children born to mothers with
hypertensive disorder of pregnancy (HDP) at the corrected age of 3 years. We compared the proportion of children
whose height did not catch up at the corrected age of 3 years between SGA children born to mothers with HDP (HDP
group) and those born to mothers without HDP (non-HDP group). We included 103 children who were admitted to our
NICU between 23 weeks 0 days and 32 weeks 6 days of gestation between 2011 and 2021, and whose heights at the corrected
age of 3 years were recorded. The proportion of children whose height Z-scores were less than -2.0 at the corrected
age of 3 years was 23.3% in the HDP group and 30.2% in the non-HDP group. The proportion of children whose
height Z-scores were less than -2.5 at the corrected age of 3 years was 13.3% in the HDP group and 20.9% in the non-
HDP group. This study showed no significant difference in the proportion of children who did not catch up in height between
the HDP group and the non-HDP group. In conclusion, it is unclear from the results of this study whether the
mother’s HDP status may be a risk for short stature in SGA children. |
