| 新生児の神経芽腫の多くは自然退縮が期待でき,比較的予後良好な疾患とされている。一方で多臓器へ転移し, 肝腫大による呼吸障害や循環障害を合併して致死的となることもある。このような症例は乳児期特有のStage MS での報告が多いが,Stage M でも起こり得る。今回,胎児期に腹囲拡大を指摘され,出生後に著明な肝腫大による 呼吸障害,循環障害,腎障害,播種性血管内凝固を認め,救命できず,病理解剖の結果,MYCN 遺伝子非増幅新生 児神経芽腫Stage M と診断した症例を経験した。Stage M かつMYCN 遺伝子非増幅であってもoncologic emergency となり得ること,また胎児期の腹囲拡大は新生児固形腫瘍によるoncologic emergency を示唆している可能性が ある。 |
| Many neonatal neuroblastomas regress spontaneously, resulting in favorable clinical outcomes. However, in some cases, the disease may metastasize to multiple organs and become life-threatening owing to complications such as respiratory and circulatory failure secondary to massive hepatomegaly. Such cases are often reported as Stage MS, which is a characteristic of infancy, but they can also occur in Stage M. We report a case in which enlargement of the fetal abdominal circumference was noted prenatally. Postnatally, the neonate developed marked hepatomegaly, accompanied by respiratory failure, circulatory collapse, renal dysfunction, and disseminated intravascular coagulation, ultimately resulting in death. Postmortem pathological examination revealed Stage M neonatal neuroblastoma without MYCN gene amplification, which can be an oncological emergency. Moreover, fetal abdominal enlargement may suggest an oncological emergency due to a neonatal solid tumor. |
