遅発性感音性難聴を発症した先天性サイトメガロウイルス感染症の1例

　遅発性感音性難聴を合併した先天性サイトメガロウイルス感染症の1例を経験した。症例は在胎39週4日，出生体重1,926gで出生し，小頭症を認め，血清CMV抗体陽性，血清・尿のCMVゲノム検査陽性より先天性CMV感染症と診断した。全身状態良好で日齢4の自動聴性脳幹反応で両側正常と判定され，ガンシクロビル療法は行わず経過観察したが1才9ヶ月時の聴性脳幹反応にて遅発性の感音性難聴と診断された。
　先天性CMV感染症児では遅発性感音性難聴の発症を念頭に置き定期的な聴力検査が必要である。先天性CMV感染症児は今後の患者増加が懸念されており，治療ガイドラインの確立，遅発性感音性難聴の予防法の確立が望まれる。

　We report late-onset sensorineural hearing loss（SNHL）in a child with congenital cytomegalovirus（CMV）infection. The antiviral drug ganciclovir had not been administered because the patient had neither life-threatening multi-organ dysfunction nor abnormal automated auditory brainstem response in the neonatal period. At 21months of age, sensorineural hearing loss was diagnosed.
　As late-onset SNHL is one of the most important complications of congenital CMV infection, the patient’s hearing should be evaluated frequently. Several studies of ganciclovir therapy for congenital CMV infection have suggested that it decreases the risk of late-onset SNHL, but it is well-known that it has serious side effects. A guideline for the treatment of congenital CMV infection and SNHL prophylaxis is needed.
