沖縄県におけるRSウイルスの流行状況と問題点

　沖縄県におけるRSウイルス（以下RSV）モノクローナル抗体パリビズマブ（以下シナジスR）の適切な投与時期を検討するため，2005年1月～2006年12月に，当院で施行した36ヶ月未満のRSV迅速診断の結果を調査した。総検体数1,844件，陽性数334件で陽性率18.1％だった。一般的にRSVは冬に流行するといわれているが，沖縄では年間を通して流行がみられ，夏季にピークとなる。近年，RSVは気温24℃以上と6℃以下のときに活性が高くなり，湿度40％で最も安定することが報告されている。これらのウイルス特性が，高温多湿の亜熱帯気候における通年性の流行に関与していると考えられる。現在は，季節に関係なく毎月の陽性数，陽性率，児の年齢を参考にシナジスの投与時期，期間を決定しているが，ハイリスク患者へシナジスRを有効に投与するため，今後も疫学データの集積を続けていきたい。

　We reviewed the results of a rapid diagnostic test of Respiratory Syncytial Virus（RSV）antigen in children under 36 months of age to confirm the epidemiology of RSV in Okinawa Chubu Hospital. The study period was from January, 2005 to December, 2006. The number of subjects studied was 1844. There were 334 positive cases（positive rate 18.1％）. The epidemic pattern of RSV in Okinawa differs from that in the mainland where RSV is more common in the winter. In Okinawa, RSV epidemics appear equally regularly and the peak incidence was in the summer. RSV activity is increased at temperatures above 24℃ and blow 6℃, and also was greatest when relative humidity was near 40％. We suggested that , in Okinawa, high humidity and high temperature permit year-round transmission of the RSV virus.
　Further studies investigating the RSV epidemiology of RSV in Okinawa will be carried out.
