胎内から骨折を認めsevere SFDで出生したOsteocraniostenosisの一例

Osteocraniostenosis in a Severe SFD Infant with an Intrauterine Fracture

　Osteocraniostenosisは，細い長管骨，頭蓋骨の骨化不全を認める重篤な疾患である。X線写真で細い長管骨，頭蓋骨の骨化不全，顔面中心部の低形成，前額突出，逆V字型の小さい口，小さい鼻などの特有の顔貌，脾臓の無/低形成が主な特徴である。
　今回，severe symmetrical IUGR，胎内での骨折，細い長管骨，頭蓋骨の低形成を認めた上記疾患を経験した。X線写真，特有の顔貌などから確定診断に至った。本疾患は，典型的な症例では，生後早期～数ヶ月で死亡の経過をたどっているが，自験例は生後6ヶ月以上生存可能であった。
　また，上記症状以外に進行性の脳室拡大，眼瞼短縮，角膜混濁，および貧血などの症状も認め，経過はきわめて重篤であった。
　本疾患の最初の報告は1988年にKozlowski K.ら1）によってなされ，その後の報告は散見される程度で，比較的稀な疾患である。本邦では2007年に日高ら2）による報告のみであり，自験例は本疾患の本邦2例目の報告である。

　Osteocraniostenosis is a serious skeletal dysplasia that consists of thin long bones, flared metaphyses, hypomineralized cranial bones, characteristic face, aplasia or hypoplasia of the spleen. A typical case has midfacial hypoplasia, a small, inverted V shaped mouth, and a short nose.
　In our case, birth weight was 546g（－5.0SD）that is severe SFD. She had an intrauterine fracture of thigh bone, thin long bones, flared metaphyses, hypomineralized cranial bones. A small, reversed V shaped mouth, impossibility of closing eyes of shortened eyelids, corneal opacity, progressive enlargement of cerebral ventricles, hypocalcemia and anemia were noted. And we could not find her spleen by ultrasonography. At age 6 month, she died of infection.
